Job Description

Collate and file all information relating to patient care ie - patient report forms/diagnosis of death forms and ECG traces. To either input data manually or scan the data from all forms into the appropriate database rectifying any anomalies that may occur and to ensure the scanner is operating effectively reporting any defects to the Clinical Excellence Manager. To verify details identified by the scanning process as requiring confirmation where electronic data To collect outcome information for cardiac arrest patients by liaising with hospitals and updating relevant databases. To maintain up to date records of staff's clinical identification numbers/personal numbers and to provide a point of contact for staff in cases of enquiries. To maintain within clinical audit an indexed system of scanned forms ready for destruction. On a regular basis audit for accuracy inputted data in accordance with existing procedures. Check patient report forms for date and station ID numbers to ensure accurate archiving. Sort patient report forms into date order for the purposes of quick retrieval on request. Where manual systems exist identify and extract any patient report forms that require archiving for 25 years, ie children, obstetric and maternity cases and archive separately. Apply current regulations regarding patient confidentiality, Caldicott Guidelines and other record management procedures. Liaise with external archive company for storage of archived boxes of patient report forms and as requested arrange for retrieval of specific boxes for the purposes of obtaining identified patient report forms. Extract all patient report forms which will contribute to any current Trust internal audit projects. Participate in national and local audit projects ie Myocardial Infarction, Cardiac Arrest, by identifying cases for inclusion from patient report forms and by searching Command and Control for any additional information. Maintain an efficient and professional working relationship with all members of staff. Promptly deal with all matters that require urgent attention. Refer enquiries on to Managers for specialist information advice and guidance. Essential Criteria: Excellent data entry and keyboard skills Attention to detail Knowledge of all Microsoft packages and using a scanner Excellent communication skills and ability to relate to a wide group of people Current enhanced CRB within the last 6 months would be an advantage This post is subject to an enhanced CRB Adecco are an equal opportunities employer
